Custom Benefit Representative at Northwest Campus As Open Enrollment Begins

October 11, 2019 – A representative from Custom Benefit Service, Inc. will be on the Northwest Campus Monday, Oct. 21, 2019, from 10 a.m. – 2 p.m. If you currently have a cancer or a life insurance policy through CBS and would like to make changes or if you would like to enroll in one of the Allstate policies, please contact SF's Human Resources office at 352-395-5191. At the appointment, you will also have the opportunity to ask questions regarding your Flex plan.
If you wish to participate in the FLEX Medical or Dependent Care plans for 2020 you must do so by logging onto the FBMC enrollment portal(Opens in new window) during SF's annual Open Enrollment(Opens in new window) period, which runs Oct. 21 – Nov. 1, 2019.
